Providing feedback – new form

You can provide feedback on any of the nine services by completing the form on the Feedback – SMART Centre page of the website. The form was recently updated following a review by our SMART Patient Focus Public Involvement (PFPI) Group (information on the group is avalable on the Get Involved – SMART Centre page). Feedback cards are also available at the centre.
The feedback is reviewed and passed on to the service, and a response is provided if requested. The feedback is collated, monitored and addressed to help improve all the services we provide.
